Immunizations and Screenings

IMMUNIZATIONS

The requirements indicated here are determined by the State of Texas and are applicable to students in public as well as private schools. Parents are encouraged to discuss immunizations with their child’s physician to determine the need for each of the indicated immunizations. Exemptions from these requirements are permitted when received in writing 1) from a physician stating the vaccine is medically contraindicated for the student, or 2) from parents indicating their decision not to immunize for conscience or religious reasons. The parent’s form must be notarized.

Each student must have a copy of his or her immunization record on file in the school office showing that he or she is up to date with all required immunizations. Immunization records are required at the time of original enrollment and updated as needed. Students without complete immunizations records or a current notarized exemption form on file are not permitted to attend class.

SCREENINGS

The Texas Department of State Health Services requires students in designated grades who attend public, private or charter schools to be screened for specific conditions that could impact their ability to learn or future health. Vision and Hearing screenings occur in grades KP, K, 1, 3, 5 and 7 as well as new students in grades 2, 4, & 6. Scoliosis screening occurs for girls in grades 5 & 7 and for boys in grade 8. If you do not wish the school nursing staff/certified screener to perform the screening, you may substitute a professional exam by a health care provider or provide an affidavit requesting an exemption.
